HearthStone Respite Care Project of Sheboygan County
The Need for Respite
In 2003, results from 450 Sheboygan County families showed that despite over 400 families with developmentally disabled members being fully served, there were still a large number of families who were not accessing services or were being underserved in the area of respite.
HearthStone has identified respite care for individuals with developmental disabilities and their families as one of its primary objectives.
HearthStone has collaborated with the Family Resource Center of Sheboygan County and the Sheboygan County Department of Health and Human Services to provide a respite cost reimbursement program for families with developmentally disabled members. The goal of this program is to positively support your family by helping to provide a much-needed break.
How the Respite Care Project Works
Contact the Family Resource Center to apply. An interview is completed over the phone or at one of the FRC sites. At that time eligibility is determined. When your family qualifies for respite reimbursement through the application process, you are given or mailed a packet of information.
- The family chooses the provider for their family member. The provider can be an existing caregiver, friend, neighbor, family member, etc.
- In the event that the family doesn't have a preferred provider, there is a provider list included in the information packet from which to choose. All the providers on the list have undergone a background check and their preferences/special skills are noted.
- The family trains the provider if necessary.
- After respite has been provided, the family submits a claim for reimbursement.
- Reimbursement is paid to eligible families at the rate of $7.00 an hour, as long as funding is available.
This is a reimbursement program, not direct service.
Who Qualifies
This project is based on:
- The need for respite service
- The individual's disability and/or availability of existing services.
If your family has been denied other services, been put on a lengthy waiting list, or has used all available respite funds, then call to apply for respite reimbursement today.
